Abstract

This study used the hospital information system retrospectively. We recruited consecutive patients with a primary or recurrent breast carcinoma who underwent F- FCH PET-CT then followed with 18F-FDGPETCT imaging prior to biopsy. All patients had Birads 4/5 breast lesions on the mammogram. Biopsy results were recorded, and the patients were dichotomized into malignant and benign groups and HER-ve and HER +ve genotype. Semiquantitative analysis utilizing the standardized uptake value (SUVmax (g/dl)/g/dl) was used to predict the two groups of variables for cellular breast cancer aggressiveness. Quality of life (QOL) assessed using the SF-36®, SF36v2®, SF-1,2® and SF-12v2® Questionnaires which are trademarks of Medical Outcome Trust are used under license. The SF-25v2®Heal is copyrighted by Quality Metric Incorporated. It later incorporated into for QOL domains – Global Health Status (GHS), Physical function (PF), Role Function (RF) and Social function (SF) according to The EORTC quality of life questionnaire (QLQ) which is an integrated system for assessing health related Quality of life (QoL) of cancer questionnaires which is validated by trademarks of the Medical Outcomes Trust. There were 21 female s with mean age of 52.82 ± 10.71 years. Eighteen patients had malignant disease (18/21; 85.8%) on histology with eleven (52.4%) with HER –ve genotype. In this study, we found that significant different between 18F-FCH SUVmax (g/dl) of the HER-ve and the HER +ve positive group (1.99 g/dl vs 0.2g/dl; p<0.05). The malignant group has higher SUVmax (g/dl) value as compared to the benign group. High SUVmax (g/dl) FCH predicted malignant breast lesion for the malignant histological subtype and the HER-ve protein overexpression genotype at the cut-off value of 0.75 (p<0.05), hence predicting the aggressiveness of breast carcinoma. 15 patients being followed up prediction of categorised FCH – High or Low group with on the QoL domains; General Heath Status (GHS), Physical Function (PF), Role Function (RF) and Social Function (SF)at 6 months and 24 months. The prediction value of FCH > 0.75 also been used for QOL prediction. We found that value of FCH primary tumour of > 0.75 which indicate breast cancer could dichotomise group of patient with and physical and social function satisfaction at 6 month with significant correlation with value of 8.067 and 5.4; p<0.05). (p<0.05) and However, it was not significant at 24 months follow up. In conclusion, the SUVmax (g/dl) FCH PET-CT of > 0.75 was a good predictor to signal an aggressive breast carcinoma cellular gene reprogramming. The usage is also significant in predict the physical and social function satisfaction at 6 months.
